Office of Community Choice Options

Through the Community Choice Program, this office works with nursing home residents, their families and nursing home discharge planners to help residents return to the community with necessary services in place to ensure their safety and support their independence.

The office also administers the Pre-Admission Screening and Preadmission Screening and Resident Review (PASRR) programs through which seniors and individuals with disabilities are evaluated to determine if they are clinically eligible for long-term care services, whether those services are delivered in an institutional or community setting.
